Ping An Insurance's own vehicle maintenance/third-party insurance

first step :   Please complete the "Claim Form and Authorization Letter" and send it to our company within 21 days of the accident.

Step 2 :   After receiving our reply, please submit the following documents as soon as possible

Original document

Completed claim form and original authorization letter

*The driver's signature must match the signature on the police document.

**Please write "Unclear" or "Not applicable" for any questions you cannot answer. Do not leave any blanks.

The above documents requiring original copies must be returned to our company by courier or post . Other documents/information can be submitted through the following link.

Other documents/information

  1. 維修估價單 (只適用於投保類別為「綜合保障」之保單) (須得保險公司同意才可維修自車，如無需保險公司負責自車的維修，請在 "索償表格" 簽名欄旁註明)

  2. 司機的身份證

  3. 司機的駕駛執照

  4. 車輛登記文件正面及背面 (俗稱 "牌簿") 

  5. 車輛牌照 (俗稱 "行車證") 

  6. 酒精測試報告 (如沒有做測試，請在 "索償表格" 簽名欄旁註明)

  7. 擬控告通知書#

  8. 警察口供紙 (若認為事故責任在對方，請通知警察並要求投訴對方) #

  9. 警察調查結果#

  10. 法庭傳票#

  11. 法庭判決結果#

  12. 除上述各項外，其他與此次意外相關的文件#
