first step :   Please complete the claim form and send it to us within 14 days of the accident .

  1. Two color photos before glass repair   (The first photo must show the glass and license plate, and the second photo must clearly show the damaged area of the glass)

  2. Two color photos of the glass after repair (the first photo must show the glass and license plate, and the second photo must clearly show the repaired part of the glass)

  3. Front and back of the vehicle registration document (commonly known as the "registration book")

  4. Vehicle license plate (commonly known as "driving permit")

  5. Driver's ID card (not required if there is no driver)

  6. Driver's license (not required if there is no driver at the time)

Step 3 : Submit original documents

  1. Original completed claim form

  2. Original receipt for glass repair fee (invoices are not accepted)

The above documents requiring original copies must be returned to our company by express delivery or post.
