Japan Property Insurance Windshield Protection​

first step :   Please complete the claim form and send it to us within 14 days of the accident .

Step 2 :   Submit your documents via the link below

  1. 玻璃維修前的兩張彩色照片 (第一張照片須展示玻璃及車牌，第二張照片須清楚展示玻璃受損的部位)

  2. 玻璃維修後的兩張彩色照片 (第一張照片須展示玻璃及車牌，第二張照片須清楚展示玻璃被維修的部位)

  3. 車輛登記文件正面及背面 (俗稱 "牌簿") 

  4. 車輛牌照 (俗稱 "行車證") 

  5. 司機的身份證 (如當時並無司機駕車則無需提供)

  6. 司機的駕駛執照 (如當時並無司機駕車則無需提供)

Step 3 : Submit original documents

  1. Original completed claim form

  2. Original receipt for glass repair (invoices are not accepted)

The above documents requiring original copies must be returned to our company by express delivery or post.
